Grant Report Forms- ALL GRANTS
If you have received a grant from the Community Foundation prior to calendar year 2012 (i.e. you submitted your application in hard copy)- please download the appropriate Grant Report Form below. If your grant was for a capital/renovation/equipment project, use the CAPITAL form. Use the General form for all other types of grants (programs, services, events, etc.)
If you need to submit a report for a grant that you submitted ONLINE, please click on the Online Grant Application and Report link below to submit your Report online as well. In the future- all grant applications and reports will be submitted online!
